Primary Care Membership Program
ACCESS TO AFFORDABLE quality healthcare
For just $87.50 a month (or $1,050 annually with a 12-month commitment), members gain predictable, reliable access to essential medical, dental, and behavioral health services under one roof. The Primary Care Membership Program is available to individuals of any age who do not qualify for, or cannot afford, other healthcare coverage options, including:
NSCHC's Sliding Fee Discount Program
Medicare or Medicaid
Pennsylvania's Health Insurance Marketplace (Pennie)
Employer-sponsored health insurance
Individuals who have only catastrophic health insurance coverage may also qualify.
By making preventive care and ongoing treatment more affordable, the program is designed to help patients manage their health, address concerns before they become more serious, and potentially reduce unnecessary emergency room visits.
WHAT DOES THE PROGRAM INCLUDE
The program offers a broad range of services across NSCHC's three primary areas of care:
-
Annual wellness visits, physical examinations, sick visits, treatment of minor injuries, chronic disease management, pediatric vaccinations, one annual flu shot, and nurse advice and triage services.
-
Routine and periodontal cleanings, dental examinations, X-rays, and cavity prevention.
-
Up to one counseling or therapy session per week, including assessments and treatment for anxiety, depression, ADHD, stress, trauma, grief and loss, and substance use concerns.
Understanding What Is Not Included
While the program covers many essential healthcare services, it is not health insurance and does not cover all medical or dental expenses.
Certain dental procedures, including cavity fillings, dentures, extractions, root canals, and crowns, are not included. Services provided by outside organizations or third parties, such as laboratory testing, bloodwork, prescription medications, diagnostic imaging, specialist visits, medical equipment, and emergency room care, are also excluded.
NSCHC may provide additional services or referrals when needed, but services outside the program's coverage may result in additional charges.
Membership does not provide priority access to providers or appointments. All participants follow the same scheduling procedures as other NSCHC patients.
To learn more about eligibility or apply for the program, contact NSCHC's Benefits Enrollment Coordinator at 412-321-4001, extension 263, or email NSCHC@NSCHC.ORG.
