NSCHC Named a Patient-Centered Medical Home by NCQA

North Side Christian Health Center has been awarded recognition by the National Committee for Quality Assurance (NCQA) as a Patient-Centered Medical Home.

“NCQA Patient-Centered Medical Home Recognition raises the bar in defining high-quality care by emphasizing access, health information technology, and coordinated care focused on patients,” said NCQA President Margaret E. O’Kane. “Recognition shows that North Side Christian Health Center has the tools and resources to provide its patients with the right care, at the right time.”

 The NCQA Patient-Centered Medical Home program reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP) and American Osteopathic Association (AOA) and others.  It was developed to assess whether clinician practices are functioning as medical homes and recognize them for these efforts. The NCQA Patient-Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement.

North Side Christian Health Center (NSCHC) was founded in 1993 when three physicians, Dr. Todd Wahrenbeger, Dr. Mark Guy and Dr. Daniel Holt, answered a spiritual call to live their Christian principles through healing work. NSCHC offers medical, dental, and behavioral health care to those most in need. Motivated to live our Christian principles, the mission of the North Side Christian Health Center is to provide whole-person primary health care to underserved persons on the Northside and surrounding neighborhoods. We envision a community where people of all backgrounds are actively engaged in maintaining a healthy lifestyle.
