If you have Gateway, Unison (Med Plus), or Aetna health insurance, you will need a referral in
order to see another physician or to get a procedure such as an x-ray or colonoscopy. You must
schedule an appointment with the physician or facility and then call the Health Center for a
referral. You will need to provide us with your:
1. Insurance identification number
2. The specialist's phone and fax number
3. The specialist's provider number
A referral request should be submitted at least a week prior to the appointment.
For tests such as MRIs, MRAs, and CTs, an authorization is required for all insurances. This
means that the Health Center must contact the insurance company and get verbal approval for
the tests. This procedure may take several days, so call as soon as you schedule the test.
